Goal Assessment and Plan
Upon completion of the assessment the client felt overwhelmed about all that needed to be done to bring some order back into her life. The Client and I agreed to design a plan that will guide her towards her goals of getting custody of her children back.
Client: Ms. Lindblom I don’t know what to do to get my children back, but more than anything else in this world I want them back.
Ms. Lindblom: What do you think we need to do to make that possible? Client: I know that I need to get myself together you know, I mean get clean from drugs and petition the courts for custody. Ms. Lindblom: that’s correct, what do you think should be our first course of action? Client: Can you write a letter to child protective services?
Ms. Lindblom: If I were to write a letter today what do you think I would be able to say? Client: I don’t know, you could tell them that I really want my kids back.
Ms. Lindblom: It doesn’t work like that what we need to do is to have proof that you really want your children back in your custody, we can start by getting you into a rehabilitation program that you will need to complete, while you are there we can make mention of your intentions to the child protection agency, this will make them aware that you are working towards getting custody, how does that sound to you? Client: that’s fine; will I get them back after that? Ms. Lindblom: More than likely child protection services will ask the judge to have you complete some parenting skills classes which are usually once or twice a week for 4 to 6 weeks, how does that sound to you? Does that sound like something that you would be willing to do to regain custody? Client: I am ready to do whatever it takes, it seems this is going to take longer than I expected. Ms. Brown: You mentioned that you had a twin, Sorry about your loss, were you close or would you rather not talk about that?
Client: Yes we was close, just like my children, and now I don’t have anyone, but I rather not talk about it, I want to talk about getting my kids back.
Ms. Lindblom: okay that’s fine Ms. Smith I think the first thing we should do is get you on public assistance for now until you are able to pursue a job, so that you will be able to eat and go to the doctor, this will also be your means of paying for rehabilitation how does that sound to you?
Client: that sounds fantastic, I really do want to go back to work, but do you think that will take a long time I know it usually takes about 30 days for public assistance, I’m not gonna last 30 days in the streets
Ms. Lindblom: In certain cases we can get you emergency assistance until your regular assistance kicks in, shall we begin?
Client: Yes, but this seems like a lot to do how am I supposed to keep up with all of this? Ms. Lindblom: What we can do is write up a contract that will include everything we discussed and take everything step by step until you have the children back. Once that’s done you will be required to work, The Department of Human Services has programs that can help you brush up on your skills and get a job, I know it sounds overwhelming right now but we will take things one step at a time, I am here to help you. How do you feel about what we discussed? Do you need some time to think things over? Client: No I don’t need to think things over, I feel a little scared, But I’m tired of the way I’m living and I just want my kids back. This is not fair to them or my mother she’s old and sickly. Ms. Lindblom: Okay let’s get started.

Contract
Upon completion of interview and assessment the Client Ms. Freydia Smith agrees to take the steps listed to regain custody of her children and to obtain a job. 1. The Client agrees to enter into and complete a drug rehabilitation program. I will find a program that will assist her with her needs. The client also needs to see a medical doctor and has agreed to seek psychological treatment if necessary. The client will need to complete rehabilitation before moving further to regain custody, if the client does not complete rehabilitation she will not regain custody of the children. 2. The client agrees to abide by the rules and regulations of the child protection services in taking Parenting skills if required. I will assist the client and child protection services in the supervised visitations based on evaluation results of the client’s progress. If the client does not comply with the desires of child protection services she will be denied visitation until further notice. 3. The client has agreed to meet with me once a week to track her progress, and assess any issues that may occur. Failure to do so may result in further investigations. 4. The client agrees to actively seek employment upon completion of the job search program as a requirement of the Department of human services. I will keep track of her progress through Ms. Lindblom case management work, and Ms. Smith. The client is advised that while on public assistance she must actively seek work, after which her time limit will expire.

Evaluation
Ms. Lindblom: Hello Ms. Smith you are looking wonderful how are you feeling? How is everything going?
Client: I’m doing great, I finally finished the program and I go to meetings. My health is good but the doctor said I have to relax, Ms. Lindblom I am so excited and scared at the same time because I have my first visit with my kids today
Ms. Lindblom: that’s great Ms. Smith it seems your doing fine, I understand your excitement but tell me what makes you feel afraid about seeing your children.
Client: well we’ve been separate for almost two years now, they call me and ask when we gonna be together again?
Ms. Lindblom: It’s important for them to understand that you have to be well enough to take care of them. Do you think that they can understand that?
Client: Yes I do. I told them it will take a little more time but not to worry because I’m working on it.
Ms. Lindblom: Good and you’re doing a great job I am so proud of you.

The client Ms. Smith has entered and completed rehabilitation and parenting classes and is currently attending NA meeting regularly. She has been granted visitation rights and looks to be in very good countenance. The client has been displaying an adamant attitude concerning staying on track to regaining custody of her children. The client has started to work on her resume. The Client has remained on track and has been complying with the contract so far. She was prescribe Tylenol for her headaches and body aches.…...

...office. She has also given me permission to interview her family members. I have also discussed with her that I should be counseling her children as well due to the sudden uprooting. Freydia is very compliant with my suggestions and seems open to any idea that helps her work towards her goals. Each week we will meet to discuss our goals and make any changes that are needed. Each week we will mark her progress and with each completed task I believe she will begin to feel less hopeless and more hopeful for the future. If at any time her goals seem unattainable we will come back and rethink her objectives. She will not hesitate to ask for help if needed. We have both signed statements of confidentiality and she knows that I cannot break this agreement unless given permission or if she plans to do someone harm. Freydia know that if I fail in anything that I have agreed to that she is free to seek help from another clinician. She also knows that if she fails to reach her goals then she will lose her children for good. Freydia feels the stress of life but she is beginning to feel in control. She has openly admitted to looking forward to the future and has more of a positive attitude. We both know that if all objects are completed and her goals met than her quality of life, as well as her families, will increase....

Hypothetical Working Agreement

...Hypothetical Working Agreement Cynthia Boland BSHS 322 December 24, 2012 Tracey Newman Hypothetical Working Agreement This paper is meant to focus on a hypothetical working agreement for one of the specified scenarios from our textbook. The working agreement should have four parts; Assessment, Goal setting, Contacting and Evaluation. This paper will state clearly the key elements and purpose of each of the four parts in the working agreement using this specific case scenario. The case that I chose to use from the text is the case of Thomas, a 20-year-old college sophomore who has dropped out of school. He has just been diagnosed with schizophrenia. His mother has brought him to the day center saying she hopes that “somehow he will be able to get back to college.” Assessment This is the stage when elaboration and exploration skills are used in assisting in assessing the story from the client. “Gathering and analyzing of information about a client is done at this stage in order to tackle the problem” (Murphy, B. C., & Dillon, C. (2011). It is also good for clinicians to do a structured assessment formally and reasonably to fulfill the purpose of diagnosis, plan treatment and carry out research on the best specialized services that are needed by the client. Assessment The purpose of an assessment is to get a vast amount of adequate information about the client. This can include basic information such as name, age, birthdate, address, and telephone number. It......

...Running head: HYPOTHETICAL WORKING AGREEMENT Hypothetical working agreement Libby McGrath University of Phoenix Communication skills from the Human Services Professional 322 Connie Giles April 29, 2013 Hypothetical working agreement Freydia, a 27 year old mother of two has been addicted to crack for 4 years resulting in her children being removed from her home be child protective services. They are currently living with her mother, and Freydia would like to regain custody of them. Since Freydia, is new to me I have to take the time to get a good assessment of her, does she really want to quit using? Does she really want her children back? How much is she willing to do to get her children back? The assessment is not something I can do just by listening to her, I need to watch closely and pick up on her non verbal gestures. By paying attention to both verbal and non verbal conversations as a clinician I can learn a lot about Freydia and evaluate her and what goals would be easiest for her to obtain at this time. After determining what goals works best for her we are now able to form a contract of sorts to go along with our goals prioritizing which might be more important than the next. After evaluating and assessing Freydia I have decided that making rehab a priority, would be the most important goal for Freydia to set, she would need to be sure to attend every meeting, and if she doesn’t think she can do that, she might have to become a resident in an......

Tort Hypothetical

... The Control Test shows an employer-employee relationship existed, prima facie, as GIC presumably had control over Colpo in performance of work, provided work equipment, recruited Colpo and paid his wages. Thus, referral to 'shortage of workers' and the unsecured ladder, suggests GIC was in breach of the duties to employ competent employees, implement a safe system of work and provide adequate equipment. This is a question of fact, with GIC judged against the standard of a prudent employer. GIC, through delegation to Mona, didn't ensure proper employee recruitment practices and Mona was possibly unqualified. GIC potentially breached his duty to provide a safe system of work by failing to implement effective supervision while Colpo was working on the premises performing purposes of employment. Lastly, the ladder may have been defective. If GIC was in breach of these duties, the 'but for' causation test is used, decided on preponderance. Evidently, GIC's breach of duty was partially responsible as sufficient safety equipment, procedures and qualified workers would have prevented workplace accidents. However, Mona's negligence was another cause of Colpo's injury. Thus, Colpo could also make a claim for vicarious liability against GIC. Mona, the tortfeasor, was GIC's employee for 'many years' and a duty of her work was to hold the ladder in question. The close connection test suggests Mona negligently performed a duty authorised by GIC during the course of employment. An......

Agreement

...I have to agree that it was Letisha’s responsibility and obligation to have read the entire contract before she entered into the agreement. If she did not want to read the contract, she should have asked a series of questions in which the salesperson for Sudson would have been obligated to answer them honestly. One of those questions should have been how the renewal and termination procedure is conducted after five years. I think Letisha may have some legal arguments that she could present in court if she gets sued for breach of contract. One of these arguments the contract is unconscionable. This contract stating that the renewal had to be canceled 90 days prior to the contract ending and by certified mail is an acceptable clause, however, to automatically renew this lease for three five-year terms or another 15 years is ridiculous. Letisha owns a five unit complex in which all the tenant use one washer and dryer, the amount of wear and tear on these machines over a 20 year period in total it is unlikely that these machines would still be working at that time. One other argument is that the contract is indefinite in which these weren’t the terms that Letisha agreed upon when signing this contract. The arguments for Sudson in support of the enforcement of the automatic renewal clause would to me be the six requirement elements of the formation of a contract. The contract was a legal offer that was made in a legal nature. Letisha agreed to and accepted the contract...
